Order by Fax

I have a fax machine online 24 hours a day at 520-219-0610. You can fax your order to this machine at any time. Please provide the following information:

• Your email address. Please try to spell it correctly.
• Credit card type (Visa, Mastercard, Discover, American Express)
• Credit card number. This is usually, but not always, a 16-digit number broken into four 4-digit groups (i.e. 1234 5678 9012 3456)
• Credit card expiration date. This is usually in mm/yy format (i.e. February, 1998 = 02/98)
• Your name, exactly as it appears on the card
• Your billing address, city, state, and zip code

Please try to write as neatly as possible. I am notoriously bad at reading people's handwriting, so please make it easy for me!

Note: I have been having a few problems with the fax machine on this phone line. If you have trouble faxing to this number, then please send me an email at smbaker@sb-software.com and I'll check the connection.

Once I receive this information, I will process it manually and send you your code back via email. If you don't hear from me in a day or so, please send me an email at smbaker@sb-software.com, and I'll run over to the fax machine and make sure it's there.
